Voluntary
Dental Insurance allows employees to purchase Dental Insurance Coverage
for themselves and families. Employees pay 100% of the premium.
Generally, Voluntary Plans are more costly than group Dental Plans
because there is usually higher participation within a plan in which
the employer contributes, and carriers favor this. However, the
covered services under Voluntary Plans are very comparable to Group
Plans so employees still have access to great coverage. In addition,
this coverage is available to groups with as few as one employee.

Benefits are divided into four sections of coverage, usually designated
as Coverage A, B. C and D or Type 1, 2, 3 and 4.
The waiting period that needs to be satisfied before employees can
receive coverage for services varies with each type of service required
but, generally, the more expensive the procedure, the longer the
waiting period. The following is a brief description of each different
coverage type.
Coverage
A (Type 1) usually
pertains to Diagnostic and Preventative type services including
examinations, x-rays, cleanings, fluoride treatments, space maintainers
and sealants.
Coverage
B (Type 2) usually
pertains to fillings, oral surgery, endodontics, periodintics, denture
repair, emergency treatment, and similar procedures.
Coverage
C (Type 3) usually
pertains to prosthodontia type services, including bridges, partial
and complete dentures, rebase and reline, crowns, and onlays.
Coverage
D (Type 4) usually
pertains to orthodontia. Generally, this benefit is available for
children up to age 19 but adult orthodontia can usually be purchased
for an additional fee.
