Click on "Report A Loss" and "Claim Notification" then complete the appropriate form  

If you suffer an emergency type of Loss, such as a fire to your home which has left it unlivable, and it is after normal business hours, use our 24 Hour Emergency Loss Reporting Service. You can reach the Service by calling 800-482-7443. The phone system will prompt you to leave a message. After you leave a message in Concords Emergency Message Box their “On Call” Adjuster is notified and will call you back.
